My take would be there wasn't a good cross hatch hone on cyl walls to hold oil.Also may have been from cold start ups .

I think it is lack of oil because the damage is on the thrust side of the piston.

Other piston is probably scuffed on the thrust (rear) side as well

Hot Idle speed on an Ironhead is 950, cold is higher ~1500 rpm.

Or perhaps no oil? or no oil feed?

The piston pictured does not appear to be very old.

I just looked them up, they are Wiseco forged pistons. No I wasn't there when they were put in and know nothing about them, but I will say I'm ticked okay no bullshit that they are virtually new and yet wrecked.

I do believe the cylinders are able to be cleaned up...I don't think I noticed any cross hatching...I think I will hone them...

My best guess is air leak at the manifold...I think Shaky Dave is probably right but before it goes back together the bore should be measured at a machine shop and I will know more...I'm fixing the case crack before I deal with the cylinders so this problem will go on the shelf until then...

I'm really pissed about those pistons being destroyed...

several issues
rich mixture and cyl wall wash off
lean mixture and heat
not enough cooling
wrong oil
when you pulled the heads, oil drains to skirt open (head gasket), should be no issue at +.020 but as you bore, the skirt holes rise and if you have a crappy rifle drill, could allow the skirt to block good drain back. very rare but have seen the holes that needed tapering downward.
oil is the cooling agent here if blocked, oil goes to p/r tubes.
lack of lubrication
wrong initial fit, expansion

My best guess is overheated, either new pistons not given enough clearance or someone didn’t give them enough break in time before twisting the wick. Either way, the pistons expanded to the point where clearance disappeared.

I would have to say they look like lack of clearance, being forged they will swell a ton more, especially that being the front cylinder makes sense since it runs the hottest.
I have done that exact same thing when I put my stroker together S&S told me to clearance them 3-5 and they tried locking down. I ended up clearance at .007 after destroying 2 sets of pistons.
Just take your time it will all come together.

If the pistons were too tight the damage would be equal on the front and back side.

Damage like that could be caused by an extended idle at low RPM.

It sounds really cool to let it sit there in the driveway chugging away potato potato potato.
But that is the sound of your pistons being ruined...
