Default 86 sportster gear issue...help!!

I have an 86 Sportster that wont go into 4th gear (simply acts like neutral). I tried doing a clutch adjustment, adjusted the primary chain, and changed the primary fluid thinking one or all of those might help. However, while it now shifts perfectly thru 1-3, it still wont go into 4th. For a few weeks prior to now it would be hit or miss going into 4th. It would either simply go into 4th without a hitch, or act like I somehow went into like neutral, but upon a second attempt to hit 4th gear it would go. Now it wont at all. Any thoughts as to what this might be and how to fix it? thanks.

Hate to say it, but the trans has to come out, it's the only way see what's going on.

Take it out. Change camshaft follower if you haven't already (use aftermarket steel version) Check out forks and all. Make sure it shifts on the bench.
