ALL!!!! FF helmet , neck brace , flexible honeycombed back protector Full leathers with elbow and shoulder armour. Gloves with bits in to stop the fingers bending back.
knee protectors with anti lateral twist armour. Boots with ankle plates to stop them twisting and sole plates to prevent the foot bending to much.

Me? leathers as road rash is a bitch! Some times Kevlar lined jeans. Armour in the elbows and a decent helmet some times open faced some times full depending on the weather. And gloves & boots..

Or do by safety precautions. perhaps you mean loud pipes

Non-DOT, but anecdotally I can tell you that I'm glad I was wearing my ACC Headtrip on this day. Not so glad that my leather jacket was in the saddlebag!

No head trauma. Your mileage may vary....
